Tailored Care & Support Ltd provides high specialist Domiciliary Care and Supported Living Services for people experiencing complex mental health needs, difficulty with illicit substances and for those who have a forensic history. We also cater for individuals who have a functional mental illness with secondary diagnosis of a learning disability or autism.
We work in partnership with the clinical team to optimise mental health stability, physical well-being and recovery. The service aspires to minimise any potential risks to others by ensuring a comprehensive assessment process and an achievable support plan for the service users. The service aims to offer our clients the opportunity to live as independently as possible with the support of staff whilst maintaining an optimal level of their mental health and physical well-being.
